Our Glen Burnie office is located in the Empire Medical Building at the intersection of Oakwood Road and Hospital Drive, across the street from Baltimore Washington Medical Center.

From I-95 or I-895: Merge onto I-97 South, exit 4, toward Annapolis/Bay Bridge. Merge onto MD-100 East exit 14A, toward Gibson Island. Take the Oakwood Road exit. Turn right onto Oakwood Road. Turn right onto Hospital Dr.

From MD-100: Travel west toward Baltimore. Take Oakwood Road, exit 15. Turn left onto Oakwood Road. Turn right onto Hospital Drive.

Our Hanover office is conveniently located across the street from Arundel Mills Mall.

From I-695 W: From I-695 W, take I-97 S exit towards Annapolis/Bay Bridge. Take exit 14B to merge onto MD-100W towards Ellicott City. Take exit 10 for Arundel Mills Blvd/MD713 towards Hanover. Turn left onto Arundel Mills Blvd., then left onto Ridge Road and another left onto Teague Road.

From MD-100: Go east on MD-100. Take exit 10 for Arundel Mills Blvd. Turn right onto Arundel Mills Blvd., then left onto Ridge Road and another left onto Teague Road.

Insurance, Payment and Billing

At Chesapeake Orthopaedic & Sports Medicine Center, we accept Workers’ Compensation, Medicare, and most major insurance plans. Please contact your insurance company or you may call our office at (410) 768-5555 or (888) 313-5209 to find out if we accept your insurance plan.

If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, and MasterCard.

Referrals

For an orthopedic visit: If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit. Please coordinate this with your primary care physician in advance. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit.

For physical therapy: You will need a referral from your physician or medical provider (primary care doctor, nurse practitioner, orthopedic surgeon, podiatrist, chiropractor, etc.) to have your insurance pay for your physical therapy treatment. 

Chesapeake Orthopaedic & Sports Medicine Center will accept an initial PT referral by fax, by phone, or by email from your provider, and we will obtain the needed signed prescription within days of your referral, or you may bring the signed referral form with you to our office.

Hospital & Surgery Center Affiliations

Doctors at Chesapeake Orthopaedic & Sports Medicine Center have hospital or surgery center affiliations with Baltimore Washington Medical Center and the Surgical Center of Glen Burnie.

Prescription Requests

Prescriptions and refills for established patients of Chesapeake Orthopaedic & Sports Medicine Center are routinely issued during our regular office hours (see above). You can make your request by calling our office at (410) 768-5555 or (888) 313-5209. No refills are issued during the evenings, weekends, or holidays, as the doctors do not have access to your medical records during those times. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy.
